Community Care Fund Free Cervical Cancer Vaccination Pilot Scheme, FPAHK

 

The Family Planning Association of Hong Kong (“FPAHK”) requests the assistance of the press to post the following notice as soon as possible:

The Family Planning Association of Hong Kong (“FPAHK”) requests persons who called its Mong Kok Youth Health Care Centre (“MKYHCC”) during the period from 6 January (Saturday) to 17 January 2018 (Wednesday) (FPAHK ref. no. 7309 – 7413) for appointments under the “Community Care Fund Free Cervical Cancer Vaccination Pilot Scheme” (“Pilot Scheme”) to contact the FPAHK at 29197878 or 27704993. 

A list of persons who called the MKYHCC or requested through the online system to make appointments with the MKYHCC under the Pilot Scheme during the above-said period was found missing on 29 January 2018.  FPAHK has lost track of those persons who made phone calls for appointments.  FPAHK has reported the incident to the Police and the Office of the Privacy Commissioner for Personal Data, Hong Kong.  As a precaution, persons concerned are advised to stay alert to unknown telephone calls and contact the FPAHK in case of any questions through the telephone numbers above. 

FPAHK deeply apologises for the inconvenience caused to the affected persons.  To avoid recurrence of similar incidents, FPAHK has drawn up a set of improvement measures for its staff members’ compliance.

A list containing personal data (including names, contact telephone numbers/email addresses, age and beneficiary categories) of 105 persons who requested for appointments at the Mong Kok Youth Health Care Centre (“MKYHCC”) during the period from 6 January (Saturday) to 17 January 2018 (Wednesday) was found missing.  Within this list, 23 persons had submitted their requests online and therefore their contact information can be retrieved.  The remaining 82 requests were made by telephone and their contact information is therefore lost.  The FPAHK has exerted much effort in locating this list but in vain. 

Apart from seeking assistance from the press, FPAHK will insert a pop up message on its website and post up notices in the three Youth Health Care Centres, namely MKYHCC, Wan Chai YHCC and Kwai Fong YHCC. 

The FPAHK is very concerned about the incident and has drawn up a set of improvement measures including guidelines for proper keeping of records for its staff members’ compliance.  Separately, briefing sessions will be held for staff members on the importance of protection of personal data and privacy.


The FPAHK, a non-profit organization, advocates, promotes and provides information, education, medical and counseling services in Sexual and Reproductive Health and Rights and Responsible Parenthood for the community.
